Case Management Support Specialist

Accountabilities

The Division of Fee for Service Management (DFSM) is looking for a highly motivated individual to join our team as a Case Management Support Specialist. This position is responsible for processes and projects related to the care management efforts for AIHP members. Major duties and responsibilities include but are not limited to:


* Will work closely with Care Management Systems Unit (CMSU) nurse case managers and other CMSU staff on coordination of care for AIHP members, including care management and care coordination support functions.


* Monitoring, processing and distribution of care management referrals, outreach, documentation and follow-up as directed.


* Will serve as the lead for care management projects, including research, development and maintenance of care management resources, processes and desk levels, and tracking and reporting of care management outcomes and data.


* Outreach to IHS/638 and non IHS/638 facilities to establish relationships and maintain facility contacts and resource data.


* Respond to provider inquiries related to prior authorization processes, denials, reconsiderations, and other CMSU processes.


* Serve as back up to other CMSU team functions.



Minimum Qualifications

* Knowledge of healthcare delivery systems, member psychosocial and medical needs, and mechanisms necessary for coordination and delivery of services.


* Monitoring, processing and distribution of care management referrals, outreach, documentation and follow-up as directed.


* Interpersonal skills in working and communicating with agency management, providers and individuals from diverse cultures and socioeconomic backgrounds.


* Proficiency in organizational skills for setting priorities, workload and record keeping.


* Proficiency in oral and written communication.


* Ability to read, interpret and apply complex rules and regulations; to work independently and as part of a team, and to know when to elevate an issue/concern.



Preferred Qualifications

* Experience in working with tribal facilities and the various health care delivery systems in which tribal members may receive services.


* Previous experience working in care coordination and familiarity with principles of case management.


* Intermediate experience with Microsoft Office Suite applications.


* Experience with research, analysis, tracking and reporting of data.
