Behavioral Health Nurse Case Manager

Job Summary:

The Division of Fee for Service Management (DFSM) is looking for a highly motivated individual to join our team as a BH Nurse Case Manager. After a successful training period, this position will work primarily from home, with some in office meetings and on-site duties being required. This position is responsible for understanding the needs of, and coordinating care for, members with physical, behavioral and CRS conditions, and other special health care needs.

Job Duties:

* Coordinates medical and/or behavioral health care for members with unusual and complex cases, including but not limited to CRS referrals, specialized transportation, discharge planning, specialty care, arranging for medical equipment, skilled nursing, or home health services.

* Coordinates services and authorizations for members transitioning from AHCCCS health plans to the American Indian Health Program.

* Works with Tribes, TRBHAs, and IHS/638 medical homes to establish rapport and close collaboration and works with other Fee-for-Service providers for care coordination.

* Coordinates medical care for transplants in conjunction with AHCCCS Division of Health Care Management (DHCM).

* Serves as back up resource to all clinical functions in the Care Management Systems Unit (CMSU).

* Develops reporting, tracking, and trending for presentations to DFSM Management.

Knowledge, Skills & Abilities (KSAs):

* Hold a current Arizona RN license.

* Knowledge of case management concepts, principles, and strategies.

* Knowledge of AHCCCS and ALTCS program design and implementation, and relevant federal and state statutes, regulations and rules governing same.

* Medical and behavioral health care management and delivery.

* Strong ability to collaborate with others for a mutually beneficial outcome.

* Analytical skills to identify and correlate specific patterns, initiate investigations, submit findings and recommendations.

* Excellent communication skills, verbal and written.

Selective Preference(s):

* Ideal candidate will have 3 years’ experience as a Certified Case Manager.

Pre-Employment Requirements:

* Hold a current Arizona driver license.

* In accordance with CFR §432.50 FFP, skilled professional medical personnel must have professional education and training in the field of medical care or appropriate medical practice. "Professional education and training" means the completion of a 2-year, or longer, program leading to an academic degree or certification in a medically related profession.

* For this position, a licensed Registered Nurse, Physician's Assistant or Nurse Practitioner is required.

* Experience in the administration, direction, or implementation of the Medicaid program is not considered the equivalent of professional training in a field of medical care.
